An observation dating back many years, from the chief of medicine at med school, now reincarnated as the Eighth Law of the Dinosaur:
The better the surgeon, the more reluctant s/he is to operate.

Yup. Agree wholeheartedly with that!
I've said many times over my career that some of the best decisions I ever made were to not operate. In fact, I'd have to say there were a few primary care docs who were annoyed when I'd send a patient back to them, unscarred. Much more than technique, judgment is the sine qua non of good surgery.
